Is there any possibility that a person with approved I-140 fails in the second step with I-485?

It is not common for USCIS to deny an I-485 after the I-140 has been approved.  The most common reasons for being denied a green card include: (1) communicable diseases (e.g., syphilis, TB), (2) criminal or terrorist activity, (3) public charge (i.e., unable to financially support yourself in the US), (4) unlawful presence in the US, and (5) misrepresentation.
